چکیده

A 15-year-old girl with osteogenesis imperfecta (OI) and multiple bone fractures since early childhood presented with occipital headache. She had blue sclera and short neck (figure 1), right hypoglossal palsy, bilateral pyramidal tract signs, and gazeevoked nystagmus. Cervical MRI showed platybasia, basilar invagination, brainstem/high cord compression, and narrowing of upper cervical canal (figure 2). Cervical traction relieved headache. OI is characterized by diffuse osteopenia and bone fragility.1 Platybasia (flattening of the skull base) with secondary basilar invagination (developmental anomaly of the craniovertebral junction in which the odontoid process abnormally prolapses upward through the foramen magnum)2 in OI results from abnormally soft bone at the cranio-cervical junction and the odontoid process protruding through the foramen magnum.3 Appropriate screening may prevent severe neurologic consequences.

Basilar impression complicating osteogenesis imperfecta type IV: the clinical and neuroradiological findings in four cases.

OBJECTIVES To describe the clinical and neuroradiological features of basilar impression in patients with osteogenesis imperfecta type IV. METHODS Four patients with basilar impression were ascertained in a population study of osteogenesis imperfecta. All four had detailed clinical and neuroradiological examination with both CT and MRI of the craniocervical junction and posterior fossa struct...

Association between joint hypermobility, scoliosis, and cranial base anomalies in paediatric Osteogenesis imperfecta patients: a retrospective cross-sectional study

BACKGROUND Joint hypermobility is a common clinical characteristic of patients with Osteogenesis imperfecta (OI), a disorder with serious comorbidities of scoliosis and cranial base anomalies. This study aimed at evaluating how prevalent joint hypermobility is in paediatric OI patients, and to find out whether it serves as a potential predictive marker of the different spinal complications; sco...

Basilar impression in osteogenesis imperfecta. A report of three cases in one family.

Basilar impression is a well-recognised though rare complication of osteogenesis imperfecta. Three patients, all members of the same family, with advanced basilar impression complicating osteogenesis imperfecta tarda, are described. The clinical features in these cases illustrate the natural history of this condition: from asymptomatic ventricular dilatation, through the foramen magnum compress...

Commentary on: “To the Occiput or Not? C1–C2 Ligamentous Laxity in Children with Down Syndrome”

Siemionow and Chou review the surgical management of atlantoaxial instability in the context of Down syndrome, using two interesting illustrative cases with different craniocervical abnormalities (atlantoaxial rotatory subluxation and os odontoideum). Both patients present with significant craniocervical spinal cord compression, and in this context, they highlight the specific case nuances that...

A case of cerebellar hypoplasia in a Chinese infant with osteogenesis imperfecta.

We report a unique case of unilateral cerebellar hypoplasia in a young Chinese girl with osteogenesis imperfecta type IV. Magnetic resonance imaging showed mild basilar invagination and impression. Although unilateral cerebellar hypoplasia and osteogenesis imperfecta may have been coincidental diagnoses, we propose possible mechanisms for unilateral cerebellar hypoplasia secondary to osteogenes...
